Vibration investigation identified inadequate design and anchoring of the pump motor base. Insufficient stiffness and poor fixation led to elevated vibration and repeat faults. The recommendations included structural stiffening, proper grouting, leveling, correct anchor bolt sizing and torque, and shaft alignment.
